How to Register for the COVID19 Vaccine

To register online you will need:

  1. A smart mobile phone, an electronic tablet, a desktop, or a laptop computer.
  2. A working connection to the internet.
  3. Your ID document, or the ID document of the person you want to register.  You can use any of the following documents: Passport, National Card [for Iraqi nationals], Civil Status Identity [for Iraqi nationals], Iraqi Nationality, UNHCR Registration Card [for asylum seekers and refugees], KRI Residency Card [for asylum seekers and refugees].

To register from the Central and Southern Governorates of Iraq

  1. On your internet browser, navigate to https://cov19reg.phd.iq/
  2. Proceed for Registration to start your application.

How to fill in the online form (Center and South Iraq)

  1. Fill in your full name as shown in the ID document that you are using.
  2. Fill in the ID number of the documents like UNHCR Registration Card [for asylum seekers and refugees], Residency Card [for asylum seekers and refugees] that you are using. Use English numbers only in this field. Do not use the separators (-) (/) and letters in your ID number.
  3. In the gender filed, select the gender option that pertains to you.
  4. In the age field, select the number that reflects your age.
  5. In the governorate of residence field, select the governorate in which you live in.
  6. [for women only] kindly indicate if you were pregnant, nursing a child, single, or other
  7. Fill in your phone number. Make sure you provide a working number so the heath authorities can reach you for your vaccination appointment.
  8. In the Resident Type field kindly select Refugee if you are already registered with UNHCR as an asylum seeker or refugee. Displaced Iraqi nationals in IDP camps should select “Displaced Iraqis”.
  9. Start first by selecting the directorate of health in the governorate where you live in, then select the health district, then select the health center/hospital where you want to get the vaccine
  10. Fill in the Workplace filed. If, however all the provided option does not apply to you, kindly select “Other”
  11. Type in your work title or profession.
  12. Fill in your Birth Date in this format Day/Month/Year

Diseases

  • Kindly answer the medical history questions, and start by answering if Did you get Covid-19? Kindly answer by yes or no, or you don’t know.
  • Chronic Illnesses and disabilities: kindly check the boxes of all diseases or disabilities that you are diagnosed with.

Finally: Press the SAVE button to submit your application.

  • You will receive a confirmation text message on that mobile number that you have provided.

To register from the Kurdistan Region of Iraq

  1. On your internet browser, navigate to this address http://vac.health.digital.gov.krd/
  2. You can choose your preferred language among English, Arabic, and Kurdish by selecting the language at the top right side of the page.
  3. Proceed to New Registration to start your application.

How to fill in the online form (Kurdistan Region of Iraq)

User Information

Fill in your personal information

  1. Select your Identity Card Type from the choices available.
  2. Fill in the ID number of the documents that you have selected. Use English numbers and letter only in this field. Do not use the separators (-) (/) in your ID number.
  3. Fill in your full name as shown in the ID document that you are using.
  4. Fill in your phone number. Make sure you provide a working number so the health authorities can reach you for your vaccination appointment.
  5. Fill in the Workplace filed. If, however, all the provided option does not apply to you, kindly select “Other”
  6. Type in your work title or profession.

  • Start by selecting the governorate where you live, then select the District, and finally the health facility nearest to you.

Diseases

  • Kindly answer the medical history questions, and start by answering if Did you get Covid-19? Kindly answer by yes or no.
  • Chronic Illnesses: kindly check the boxes of all diseases that you are diagnosed with.
  • Finally: Press the SAVE button to submit your application.
  • You will receive a confirmation text message on that mobile number that you have provided.
  • You can also find out the date of your vaccination by entering the site again and going to “Check your appointment” and then entering the ID number with which you have completed the registration for the vaccine.
