Completing your forms
All registered My Services users are invited to complete a form that asks for your information related to your protection situation and documentation. Your responses help UNHCR plan better service delivery and understand the needs of forcibly displaced people seeking asylum in Pakistan.
The data you provided will be accessed only by authorized UNHCR staff. Information collected will be securely analysed to support protection and assistance processes. The information you submit will be recorded and retained in UNHCR database. You may request corrections or updates by contacting UNHCR through National Helpline at 0800-86677.
Who receives the forms?
The form is requested for all users who have a verified My Services Portal account. It will be available immediately after you first log in to the portal and access your account. You can find it in your portal account under “My Forms.”
Once you completed the form, it will be marked ‘completed’ and you will not be able to submit another entry.
How do I access and complete a form?
1Log in to your verified My Services Portal account
2Go to the Home page
3Go to My Forms section
If a form is available for you, you’ll see it under the “My Forms” section.
4Click on the form to complete and submit it.
